Business Hours

9:00am to 5:00pm Monday – Friday

Arrival Time

Please arrive 10 – 15 minutes prior to your appointment time and bring the following information:

 

    • Forms Completed forms
    • Forms List of medications, vitamins, herbs or nutritional supplements that you take on a regular basis. Please include the dosage information or you may just bring the bottle with you and we will record them.
    • Forms Any MRI, CT, X Ray films or CD’s for your physician to review.
    • Forms List of questions you may have for your doctor.

Cancellation Policy

 

If you must cancel your appointment please call (419) 483-2403 at least 24 hours in advance.  If you fail to do so you will be charged a $50.00 fee.  If you must cancel a testing appointment, please call (419) 483-2403 at least 24 hours in advance or you will be charged a $100.00 fee.
